Voluntary: If you wish to enroll in Voluntary Life and AD&D, Short-Term Disability, Long-Term Disability, Critical Illness, and/or Accident coverage, please refer to the Guardian Enrollment Booklet and complete the application.

I and TraLo Specialized agree that beginning the first pay period following 01/01/2021 and continuing each pay period thereafter until the Agreement is amended or terminated, my salary will be reduced by the amount of my required contributions for the benefits that I have elected under this Plan. I understand that if I am terminated from TraLo Specialized t paycheck. The amount of my required contribution has been provided to me. I understand that this Agreement is irrevocable and cannot be changed until the next plan anniversary in 2022 except for certain qualifying events such as marriage, divorce, birth of a child, spouse leaving or accepting full time employment, etc. Further, any previous Compensation Agreement under the Plan relating to the same benefits offered is hereby revoked.
